Health promotion initiative
Health promotion initiative: This health promotion program especially aims at the ways of addressing the patient’s adherence to medication and interventions for diabetes. To measure adherence to this intervention, all patients under treatment should have average blood glucose for the last three months (HbA1c level) of 48mmol/mol. Elderly men patients diagnosed with diabetes should have HbA1c level is 48mmol/mol (6.5%)(Measurable) or below (Specific) during the next two months (Timely) to identify adherence to pharmacological and non-pharmacological treatment (Actionable and Realistic)
